In the text Study Guide Third Edition, (page 356) it says SCFE “May present with referred KNEE pain with a normal knee exam and a leg that is extended and internally rotated at the hip.”

In the AAP News article “Limp with knee or thigh pain? Consider slipped capital femoral epiphysis,” it states that the “child limps with the foot turned out.”

WOULDN’T THAT MEAN THAT THE LEG IS EXTERNALLY ROTATED AT THE HIP (as opposed to “internally rotated at the hip” as it states in the study guide), IF THE FOOT IS TURNED OUT???

Thanks so much for any help with this!!!

Hey Sheila!  You are correct.  That is a typo in the study guide.  The hip is externally rotated in SCFE.  The patient will not let us internally rotate, it hurts.   That’s why when they come in with knee pain, we always log roll the leg.

FWIW, most hip processes present with pain with internal rotation.  SCFE is no exception.
